ANCOM makes available to interested persons the notification form, in electronic, editable form.
The notification form must be signed by the representative of the applicant/provider, using therefor an extended electronic signature, based upon a qualified certificate not suspended or revoked at that time and generated using a secure electronic signature creation device.
